Get entries with assessments for a report + subject (legacy GET /reports/assessments/{ReportId}/{SubjectCode})

Returns one entry per pupil + entry name for the report, restricted to the subject's sets, each carrying its assessment mark-grades. Port of the legacy RPT_AS_GetEntriesForReportAndSubjectFaster SP, rebuilt against the canonical Subject/Class bridge.
