Convert a pushed app to hosted (human workspace owner only)

Points an existing pushed app at a manifest URL. The fetched document becomes a candidate the owner approves like any refresh (or version 1 directly when the app never published). From here on the app's manifest is the vendor's document and the draft editor refuses writes (hosted_manifest_read_only).
