Publisher Source Audio Attach

Attach a finished upload to one episode and mark it live.

Verifies the object EXISTS via head_object before touching the database — an episode is never marked live on the client's word alone. Idempotent: re-attaching the same key to the same episode updates in place rather than minting a second asset.
