Get Publisher Authorization

The publisher's on-file Audio Authorization acceptance + revocation state.

revoked is the effective flag — takedown_at (the always-wins revocation) OR a ledger revocation row. The dashboard Agreement card renders on_file (the receipt) and toggles the "Cancel authorization" affordance on revoked.
