Creator Claim Accept

Accept the claim and provision the managed service. Idempotent — re-posting an already-claimed token returns the SAME publisher + links (no duplicate rows, no re-fired one-shot emails). A provisioning failure after the claimed_at stamp still returns a usable 200 (loudly logged) — never the pre-2026-07-16 dead-end.
