Scan-run HISTORY, newest first — one item per ingest (mcp:*, read-only, 0 credit).

MCP twin of GET /v1/projects/{id}/code-scan/runs. Each item is one run's recap: counts{new, persisting, treated, gone}, submitted, via, actor. Cursor pagination — read cursor_next, pass it back as cursor until null.

An EMPTY history means no scan has ever run here — an unmeasured project, not a clean one. summary shows only the LAST run; this shows all of them.
