Mcp A11Y Queue

Unified a11y moderation view (overrides + gaps) — mcp:* twin of the dashboard queue. ?state=rejected lists rejected cells + reject_reason; group_by_key=true aggregates by key. a11y_untranslated (coverage) is segregated into untranslated_count by default (task 1345/AC1).
