Save User Template Parameter Details

Replace the per-user parameter/recipient rows for a PAGINATED template in LS_Lia_User_Template_parameter_details: delete existing rows for the template (LSCode- scoped), then insert the supplied rows. title/sub_title stay UI-only. Rows missing a required key (user, email, value, param name, report) are skipped.

Writes go to the CUSTOMER database — the same one paginated_work_items and the lsservicebus scheduler read. Writing to the main-DB db instead produced rows nothing would ever pick up, and the run would report "no recipients".

[Template Name] / [Report Name] / [User Name] are NOT NULL but purely denormalized, so they are looked up from the main-DB masters when the payload omits them rather than forcing every caller to send labels it does not own.
