Get User Template Parameter Details

Recipient + parameter (filter) list for a PAGINATED template, from LS_Lia_User_Template_parameter_details — a CUSTOMER-database table, so it is read through custCursorConnection rather than the main-DB db. Each row = one user with their own filter [Value]. Scoped by LSCode + template_code (template_code is globally unique; LSFirmCode is numeric so not matched here).

title / sub_title are still UI-only: the table has no such columns, but [Template Name] and [Name Description] are far better labels than repeating the parameter name, so they fill those slots when present.
