List metric failure rates for the insights window

Return the percentage of evaluated calls that failed each metric during the selected Insights date range. Optionally scope to agent_ids (comma-separated agent ids) and/or agent_version_ids (comma-separated agent version ids) — the latter is how you compare a metric's failure rate before and after an agent version change. Rates are counted from the verdicts recorded at evaluation time, so editing a metric's failure criteria changes future calls only.
