List agents implicated by a failure mode

Return the agents whose calls appear in one failure mode's example calls, with a per-agent call count. Select the failure mode with the index query parameter. Use this to choose the agent for generate-scenario or reproduce; falls back to the project's agents when no example call resolves to an agent.
