Apply Team Approval Optimization

Apply the optimizations this approval authorized.

The approval is the authorization, so this refuses anything the user has not approved. It patches the campaigns directly rather than resuming the agent team: the decision is already made, and what remains - call the provider, write down what happened - needs no model.
