Recheck Env

Drift sweep (Phase 4): re-evaluate the env's at_target processes against their live governed evidence. Any that no longer hold (faithfulness SLO breach / fitness below target) are flipped to drifted, their loop is reopened, and an operator inbox card is filed. Idempotent + debounced (one open card per process). Drive it on a schedule (a watchdog-style adapter or a cron subscription) for hands-off self-healing; call it directly for an on-demand re-check.
