Healthz

Liveness probe. 200 while the process is alive — INCLUDING during the background startup warmup (the pod is healthy, just not yet ready for data; see specs/nonblocking-startup/v0.1.md). Returns 503 only when the warmup has FATALLY failed, so k8s restarts the pod instead of leaving it permanently un-ready. Backward-compatible: ok is always present; version / spec_version are additive.
