Library Session Scrub

Re-run the secret scrubber over a saved session's transcript — account-scoped. ?dry_run=1 returns {findings, would_change} without writing. Otherwise, if the scrub changes anything, publishes a new CLEANED session version and bumps the collection manifest to point at it (the previous version stays in history).
