Mark Unserved

Operator: take a served adapter out of service — THE intentional unserve path (fleet-router #62). Removes the run's router key from the fleet table and marks every run served on it unserved, atomically. Body: {if_version: int (the fleet table version you read — required), reason?: str}. Idempotent: a run that is not served answers 200 with its state, nothing written.
