Index: every skill with its served latest version + content_hash.

Mirrors GET /polaris-baselines. Without this there's no enumeration entry point — a caller must already know a skill_id (the per-skill routes 404 on unknown ids), and a bare GET /agent-skills would fall through to the SPA catch-all and return dashboard HTML.
