Embed Click Attribution

Server-side embed clickthrough beacon. Sets the first-party ol_embed_ref attribution cookie + emits embed_clickthrough, then 302s to the canonical page. ALWAYS private, no-store (never a cache surface). See the block comment above for the COUNSEL+CISO rationale.
