Update Outbound Callback Stages

Replace the full set of outbound callback ladder stages.

Stage numbers must be contiguous starting at 1. An empty list deletes the customer's stages, which restores the defaults. Also persists the callbacks_active arm switch, which is what actually starts and stops re-dialing; editing the ladder with it off is a safe, inert operation.
