Replan Desk Decision

SPENDS CREDITS. Gather fresh evidence and write a NEW plan version.

Rep-pressed only; nothing automatic reaches this. It rides the org's existing account_plan_replan switch, the per-account 24-hour cooldown and the org's daily cap, and a press cannot bypass any of them.

A refusal from a gate is a 409 and costs NOTHING. A failure after the model answered may already have been charged, and says so in the response. There is no retry anywhere in this path, so a 409 means stop rather than try again.
