Write this account's plan, from a plan the CALLER authored.

Create and replan are the same operation because a plan is versioned: this inserts a new version and bumps plan_version whether or not one existed. Nothing is generated and nothing is charged - the steps come from the request body. For the desk to draft them instead, use /plan/draft.
