Get Application Platform Modules Endpoint

Modules an application has materialized at the platform:crisp scope.

Sourced from OpenFGA (module_instance:platform/crisp/<app>/<module>) rather than the catalog attachments policy: a platform-published semantic model can be materialized at platform scope while the catalog row records only an all-account fan-out (a companion re-registration overwrites its attachment policy). Surfacing these lets the operator manage platform- scoped models from the application's Global instance tab.
