Send Reviewed Draft

Send stored recipients/settings at a reviewed revision; retries return the receipt.

The document id plus revision is the dedupe key, including a lost response after commit. No model-generated operation key and no new grant schema. Only sessions this endpoint created (draft_reviewed_send) replay; a draft converted by another send path is a 409, never an approved receipt.
