Save the structured agenda for a session (MED-975 / E1.3d)

Full-replace save: the payload is the authoritative state for the session's (cohort, modality, session_number) scope. The service runs a transaction that soft-deletes existing sections (and cascade-soft-deletes their visuals + prompts) and inserts the payload's sections fresh. Description is upserted on session_number_config.

Per AC-MC3, Save is always allowed for in-progress work — the endpoint does NOT enforce minimum-complete (section_name and duration_minutes are nullable). The UI computes and visualizes the incomplete state.
