Get the structured agenda for a session (MED-975 / E1.3d)

Returns the description + ordered sections + each section's visuals and leader prompts for the session's (cohort, modality, session_number) scope. Empty arrays when the session has no authored content yet (AC-AS1 empty state is rendered client-side from this shape).
