Get Questionnaire Structure

Get questionnaire structure grouped by phases.

Returns a list of phases with metadata including:

  • Phase name
  • Phase order
  • Question count
  • Question IDs in that phase

If session_id is provided, also includes completion status:

  • Answered questions count
  • Whether phase is complete
  • Whether phase is current

This endpoint is used by the frontend to display phase cards in the wizard overview modal.
