Create a skill

Creates a new DRAFT skill and its first version for the authenticated caller. Authenticated. Org-scoped skills (via orgSlug) require org membership and are limited to ORG/DEPARTMENT/TEAM visibility; orgs that gate new skills queue the draft for approval.
