Record a skill installation

Records an install event and increments the skill's install counter. Optionally authenticated — authenticated installs dedupe once per user, anonymous installs dedupe per IP for 24h. Enforces skill visibility and the caller's org install policy. Body accepts optional version and platform. Returns the skill slug and the version that was recorded (the requested version, or the latest published version when none was given).
