Create a skill version

Publishes a new, higher semver version of a skill with updated instructions and optional changelog. Restricted to the author, an org PUBLISHER+, or an admin. Runs AI review; in orgs that gate modifications, a non-admin's version is held pending approval. Notifies installers of the update.
