Publisher Account - Regenerate Key

Rotates key material IN PLACE on the same publisher_account row. The account (name, cpc_share, rate_limit_tier, historical click attribution) is preserved; only the key's prefix / hash / hint change. Old key stops authenticating IMMEDIATELY. Plaintext returned EXACTLY ONCE — unrecoverable after this response, same policy as admin issuance.
