Publisher Account - Regenerate Key

Rotates key material IN PLACE on the same publisher_account row. Same semantics as the self-serve /publisher/v1/api-keys/{id}/regenerate — account (name, cpc_share, tier, historical click attribution) preserved; prefix/hash/hint replaced. Old key stops authenticating IMMEDIATELY. Plaintext returned EXACTLY ONCE. Admin-initiated rotations log actor_type='admin' on publisher.publisher_key_rotation.
