Publisher Account - Reactivate

Flips active=true on a previously-revoked account. Admin-only — publishers cannot self-serve reactivate (a revoked key is off for a reason). PublisherAuth starts accepting the key again immediately (next request lookup returns active=true). Logs actor_type='admin' on publisher.publisher_key_rotation.
