Consume human approval and submit an existing VPS reinstall

Requires exact plan_id, plan_digest and approval_id. Drift, revocation, expiry, replay and missing durable audit refuse dispatch. AI cannot grant or approve. Partial dispatch requires reconciliation, never retry. Enrolled credentials cannot bypass this through legacy reinstall.
