Revoke a credential and everything it minted

Immediate, and it CASCADES: a kill switch that leaves the children running is not a kill switch, so every credential minted by this one, however deep, is revoked with it. The response lists every id that was killed.

Revoking an already-revoked credential succeeds and reports nothing killed — a kill switch should not fail because someone pressed it twice.
