close_loop: close a loop that DID produce a signal. Requires actual_signal. Preserves actual_signal and actual_signal_at, stores the interpretation, sets lifecycle_status to resolved, emits an action_resolved history event and removes the action from active prompt groups. Use close-without-signal only when no signal ever arrived.
