Acknowledge My Document

An adult student read the school's consent document and ticked.

document_version is the version the dialog SHOWED. A version that is no longer current is refused with 409 rather than stamped onto the new text, so the record always names the wording they actually read. The IP is kept for the reason /me/assent keeps the student's: a consent with no circumstances attached is hard to stand behind later.
