Create a new SCIM Bearer token. Returns the plaintext token ONCE.

Le plaintext est `scim_<prefix>_<secret>`. Après cet appel, seul le
prefix et le SHA-256 hash du secret sont persistés — le plaintext est
irrecouvrable. À copier immédiatement et stocker dans le coffre-fort
IdP (Okta / Entra ID / etc.).

Step-up MFA exigé : action sensible (création d'un secret avec accès
SCIM v2 complet sur l'org).

Permissions requises : admin:all
