Delete User

Hard-delete a user. Cascades to claims they own, OAuth integrations, workspace memberships, and sessions via DB-level ON DELETE rules. Audit log authorship is preserved via SET NULL. Self-deletion is blocked; if the target is an admin, at least one other active admin must remain.
