Decision Receipt Slot

Free: which signed decision receipt holds a request slot, its scope (account or decider), and for a decider-scoped slot any other claim the decider key signed for the same request_id (conflicts). One slot holds at most one receipt, so a receipt whose request_slot lists only its own event id is the only one we issued.
