Assemble, verify the whole-file digest, and only then create the upload.

Order matters and is the invariant: the blob is written and verified BEFORE the ingest_uploads row exists, so there is never a row pointing at bytes we have not proven. A digest mismatch retains recovery bytes without an accepted receipt.
