Queue Outreach

Queue one outreach email per recipient (PRD_PHYSICIAN_EMAIL_OUTREACH).

Each recipient is re-checked against live state; anyone no longer eligible is reported as skipped with the reason. dry_run (the default) renders without minting links or writing to the outbox.
