Close Hs Data Request

Close a request as fulfilled or withdrawn.

Two reasons and no free text, because the reason is read by a person scanning a list rather than by whoever wrote it. fulfilled says we got what we asked for; withdrawn says we stopped asking, and a partner reading the portal should not have to guess which.
