Export Case Options

Everything the five scope pickers need, in one call.

specialties / versions are what the old two-filter surface returned and are unchanged. cases and physicians are new (PRD §2.1): the Case scope types or pastes an id and wants a typeahead behind it, and the Physician scope picks a PERSON BY NAME while the bundle it produces carries only annotator_id_hashed. Both halves come from one row so a caller cannot cross them.
