Degraded Events Api

JSON: degraded events + prior state for chart reconstruction.

"Degraded" is the user-visible health status (stricter than stress mode): a (provider, tier) sustained past the stress threshold long enough AND with a site overdue beyond the tier's max scan window. One row per ENTER/EXIT transition, written by HealthChecker._check_cluster.
