Workspace Doc Versions

Cheap change-detection poll for sync clients (e.g. the iOS app): every document's id + content hash in one indexed query, no CRDT loading. A hash differing from the client's last-seen version means 'refetch this doc'; an empty hash (doc not yet reindexed) should be treated as changed.
