Answer Page By Domain

GET answer pages (per-domain opt-in). The edge worker calls this on an R2 miss for question-shaped paths, for the /c/ thread namespace, and for GET /agent-desk/ask. A 404 carrying x-answer-pages: disabled tells the worker to fall back to the pre-existing behavior for that path.
