Add an independent recording

Adds screen, camera, or audio evidence without replacing earlier answers or recordings. Each clip is limited to five minutes. requestId connects it to a review question; replaceId intentionally replaces that contribution. New material invalidates the old assessment and preview. Camera and audio are supplemental: every unpublished task requires an explicit narrated screen walkthrough.
