Generate Agent Appearance

Kick off appearance generation in the background. Returns 202 immediately. Poll GET /api/agents/{agent_id} and check character_json.appearance_status for 'generating', 'done', or 'failed'.

When skip_headshot=true, keeps the existing headshot and only generates frontview/backview using the headshot as reference.
