Refresh From Upstream

Pull the latest definition from the source and update the local row.

422 if the skill wasn't registry-installed (no upstream to refresh from). 410 if the source has been removed/disabled — caller should re-import. The existing trust gate doesn't fire here: refresh is only meaningful for skills the caller already owns + manages, and ownership is enforced via _require_write_access below.
