Remove Member

Remove a member from a colony. Moderator only. user_id is a username or a user ID.

The shared use-case closes two guard holes this endpoint had relative to the web: the founder's membership row is protected, and a colony admin can only be removed by the founder or a site admin. Writes the remove_member audit row the web writes.
